1. Introduction
Welcome to Shropshire Day Care Centre. We aim to provide a safe, supportive, and stimulating environment for all service users, including those living with dementia. These Terms and Conditions outline the agreement between the Centre and the service user or their representative.
By using our services, you agree to abide by these Terms and Conditions in full. We encourage family members or representatives of service users, particularly those with dementia, to work closely with us to ensure a positive experience.
2. Services Provided
The Centre offers a range of day care services, including but not limited to:
Social and recreational activities: Service users will be actively involved in the planning and creation of the Centre’s program of activities to ensure their interests and preferences are reflected. For service users with dementia, activities will be tailored to support memory, engagement, and overall well-being.
Nutritional meals and refreshments: Where possible, all meals will be homemade. We can accommodate dietary requirements, choices, likes, and dislikes, provided the Centre has been notified in advance. Additional care will be taken to assist service users who may require help with eating or drinking.
Personal care support (if agreed upon): Personal care services will be delivered with dignity and sensitivity, with special consideration for service users who may experience confusion or require additional reassurance.
Transportation services (if applicable): Drivers and staff will provide a calm and patient service to accommodate the needs of all service users, including those with dementia.
Day trips: These may incur additional costs or require spending money for service users, which will be communicated at least two weeks in advance. Extra measures will be in place to ensure the safety and comfort of service users with dementia during outings.
3. Admissions
3.1 All service users are required to complete an assessment before commencing attendance to ensure the Centre can meet their needs, including any specific requirements related to dementia care.
3.2 The Centre reserves the right to review and amend individual care plans as necessary to maintain safety and quality of service. For service users with dementia, care plans will include strategies to manage memory loss, confusion, or other challenges they may face.
4. Fees and Payment
4.1 Fees are charged on a per-day basis. Charges are arranged and agreed prior to commencing services.
4.2 Additional Services:
Transport - £0.45 per mile
Bathing Services - starting from £20.00
4.3 All fees are payable monthly in arrears unless otherwise agreed in writing.
4.4 Payments can be made via bank transfer, cheque, or other approved methods.
4.5 The Centre reserves the right to review and increase fees annually. Any changes will be communicated in writing with at least 30 days' notice.
4.6 Fees may be increased following a review of the service user’s care requirements. For example, if a service user requires one-to-one support or a higher level of assistance with personal care or other needs, the Centre will provide written notice of any additional charges.
4.7 Late payments are subject to a 20% daily charge per overdue day. Repeated late payments may result in suspension of services.
5. Cancellations and Non-Attendance
5.1 Visits must be cancelled at least 24 hours in advance. Failure to do so will result in the full day’s fee being charged. This policy is necessary due to the Centre’s planning, staffing, and purchasing requirements.
5.2 If the Centre cancels a session (e.g., due to unforeseen circumstances), no fees will be charged, and any prepaid amounts will be credited or refunded.
6. Health and Safety
6.1 Service users must inform the Centre of any changes to their health or medication. For those with dementia, family members or representatives are encouraged to keep us informed of any changes in behaviour, cognition, or physical health.
6.2 The Centre adheres to all relevant health and safety regulations and maintains a clean and safe environment for all users. Additional precautions are taken to ensure the safety of service users who may experience confusion or disorientation.
6.3 Fire procedures are in place and will be explained to all service users during their initial visit. Regular fire drills are conducted to ensure preparedness in case of an emergency. Staff will provide additional guidance and support to service users with dementia during such procedures.
6.4 Service users are expected to:
Follow staff instructions regarding health and safety measures.
Use mobility aids as required and inform staff if additional support is needed.
Report any hazards or unsafe conditions to staff immediately.
Avoid bringing hazardous items or substances into the Centre.
Participate in emergency drills and evacuations when necessary.
7. Centre Environment and Security
7.1 The Centre is designed to provide a safe, comfortable, and welcoming environment for all service users. This includes:
Clean and well-maintained facilities.
Spaces designed for accessibility, including ramps and handrails where necessary.
A calm and inclusive atmosphere that promotes well-being, particularly for service users with dementia, who may benefit from familiar routines and sensory-friendly environments.
7.2 Security measures in place include:
Secure entry and exit points to ensure only authorized individuals can access the premises.
Visitor sign-in procedures to monitor access.
CCTV monitoring in communal areas to enhance safety (in compliance with privacy laws).
Regular checks of the premises to identify and address potential hazards.
7.3 Service users and their representatives are encouraged to notify the Centre of any concerns related to the environment or security.
8. Transport
8.1 All vehicles utilised for transporting service users will be maintained in good order, regularly inspected, and fully compliant with all safety regulations.
8.2 Drivers will hold appropriate qualifications and ensure the safe and comfortable transport of service users. They are trained to provide extra patience and understanding for service users who may experience confusion or anxiety during travel.
8.3 The Centre will ensure that vehicles are clean, reliable, and equipped with necessary safety features, including seat belts and first aid kits.
8.4 Service users or their representatives are responsible for notifying the Centre of any specific transport needs or mobility requirements in advance.
8.5 Service users requiring transport from the Centre must be ready for collection by 9:50 am. As the transport team may have multiple pickups, it is essential for service users to be punctual. If a service user is not ready for collection, the transport team may be unable to wait, resulting in the service user missing the visit. In such cases, the full day’s fee will still be charged.
8.6 The transport team operates with a grace period of 15 minutes either side of 10:00 am for collections to account for traffic or other unforeseen circumstances.
8.7 Staff and/or transport team are not insured to provide personal care or assistance within the service user’s home to support them in getting ready for transport, bar supporting with dressing into coats, scarf, etc .
9. Medication Management
9.1 If service users require assistance with medication management, all medications will be securely locked away and only administered by a trained member of staff as directed by the prescriber.
9.2 Medications must be provided in their original packaging, clearly labelled, and within their expiry date.
9.3 Service users who self-administer their medications are responsible for managing their own medication. If service users require verbal prompts as a reminder to self-administer their medication, the Centre's staff will be more than happy to support with this. Service users may request for medications to be securely stored in the Centre’s lockable medication cabinet.
10. Behaviour Expectations
10.1 The Centre is committed to providing a safe and welcoming environment for all service users and staff. Respectful and appropriate behaviour is expected at all times. Staff are trained to de-escalate situations involving service users with dementia who may exhibit challenging behaviours due to their condition.
10.2 Any behaviour that is deemed abusive, aggressive, or otherwise harmful to staff, other service users, or the Centre’s property will not be tolerated.
10.3 The Centre reserves the right to terminate services immediately if a service user’s behaviour puts the safety or well-being of others at risk.
11. Confidentiality
11.1 The Centre respects the confidentiality of all service users. Personal information will only be shared with relevant staff or external parties with the service user’s consent, except where required by law. For service users with dementia, family members or representatives may be consulted to ensure the service user’s needs are met effectively.
12. Complaints and Feedback
12.1 Service users or their representatives are encouraged to provide feedback or raise concerns with the Centre Manager.
12.2 Complaints will be handled in accordance with the Centre’s complaints procedure.
13. Termination of Services
13.1 Either party may terminate the agreement with 30 days’ written notice.
13.2 The Centre reserves the right to terminate services immediately if the safety or well-being of staff or other service users is at risk, including in cases of unacceptable behaviour.
14. Liability
14.1 The Centre will not be held responsible for the loss or damage of personal items unless caused by negligence.
14.2 Service users and their representatives are encouraged to arrange personal insurance for valuable belongings.
15. Amendments to Terms and Conditions
15.1 The Centre reserves the right to amend these Terms and Conditions. Service users will be notified in writing of any changes with at least 30 days’ notice.
